Sometimes those ideas are in the form of a dream, a poem, an outline. I can't get them written down fast enough because I'm afraid I'll lose them. If I come up with a title first, it restricts my writing and stifles the creativity. Other times, I can visualize a theme around a title. I don't think there's any certain time to have the title pop into your head. It could be in the middle of the writing!
